I never owned a copy of this BITD, so years ago I bought a PDF. But much like the disappointing copies of the A series (which "featured" nearly illegible scans of the maps), I'm beginning to suspect I got ripped off.
In this PDF I have maps of Tiers 1-5. I had simply assumed that the "do it yourself" aspect of the module extended to mapping Tiers 6-10 and the Undercity, but there appear to be multiple references online suggesting that these maps actually exist. On the other hand, no scans seem to exist and the only copies circulating anywhere online have been created in CC2.
So, quick question for anyone who owns the original module: Am I actually missing maps for the lower levels?

You may want to check this out bud. I seem to remember that the orginal moduel had the map in a fold out in the center of the booklett. These should be what you are looking for.

Wow... I just checked the pdf I have and those maps are missing as well... Good thing for me that B4 is one of the first modules I ever owned in a physical copy, and that I still have it. That would be frustrating indeed.

Aw, man... I just found an old Ebay auction with photos that are just blurry enough not to work, but detailed enough to reveal how freaking awesome the underground city map is.
I suppose it could be worse. I heard reports that the early versions of the Ravenloft PDF were missing all of the maps.
Thanks for point me to those CC2 maps and confirming they're accurate re-creations. Nice to have some reference for them.

If you poke about I think you'll find plenty of info on how people have populated those lower layers.
I haven't read any of it though as my bro used a lot of it when he ran The Lost City for our group last year. We're still in there, having started B10 with a different group of characters to give him a break from DMing.
Also, just read your 3 Clue Rule essay - a great read.

Unfortunately, those recreations don't include the maps of the city itself, which you are probably also missing from your PDF. One of the missing maps is a cross-sectional view of the pyramid, sub-levels, and underground cavern with the city and its catacombs. The other map is a 3D bird's eye view of the underground city itself. One of my favorite maps in any D&D product.

I scanned the missing maps from my hardcopy and inserted them into the pdf so I have a complete digital copy.
